Completion of the 2019 Ohio 4-H Achievement Record (MS WORD required) is required for the following opportunities:
(descriptions can be found in the chart above; MS WORD needed to complete this form)

Please note new categories have been added for 2019: Creative Arts (scrapbooking, art projects, cake decorating, etc.), Llamas and Communications. Market Goats and Dairy Goats have been combined into one category of Goats.
